There is so much truth in that.

With doctors and lawyers, "errors" means "malpractice" so the stakes are very high. In other industries, I think errors leads to losing customers, which is obviously not sustainable in the long run, but not the instantaneous game over that it is for doctors and lawyers.

The next step is to add direct connecting lines between the opposing stages; so, the intermediate stages in the circle can be bypassed to create internal conflict more efficiently.

I saw that being done by US Army Training and Doctrine (TRADOC) Command back the in the 1980s. ;)

hiring well, training relentlessly, and holding people accountable break that cycle.


most companies are too cheap to invest in the training, and too lazy to fire slackers. not to mention clueless on how to hire.

I read this today in Whitehouses' book, Apollo 11. It's an interesting read which also draws back the shades on how similar issues were responded to by the Russians.

I hate it when people are lazy. That tells me that I dislike it when I am. So I watch it.

I cannot believe how much effort some people put into being lazy. My cow-orker and I often marvel at the extreme length people will go to to avoid walking 20 yards to get the right piece of equipment for the job. No amount of training seems to fix stupidity :D
