Things I would try:

1.) Less caffeine - even if you don't drink "a lot," your tolerance may have gone down as you get older (I know, none of us are getting older, what am I saying?)
2.) A different pillow and/or sleeping position
3.) Very simple, basic foods for a few days--I get headaches when my meals are too salty, which includes most processed foods these days.

I'm assuming your doctor has already checked your blood pressure?

I get horrible headaches when I eat a lot of foods that contain nitrates. Bacon, lunch meat, etc. Bacon is the WORST culprit.

most doctors know shit all about chronic problems. Look to lifestyle changes, particularly something in your diet or environment you may be allergic to, but unaware of.

Have you tried stretching throughout the day or before bed? Could loosen up those muscles.

Are you drinking enough water/other fluids? Your weewee should be the colour of champagne (!) or lighter.

But doesn't caffeine resolve headaches? Most headache medication contains the stuff. Taking less, especially if you have a dependency on it, is going to make the pain a lot worse!

is there a typical progression: starts in your neck and then progresses to the temple(s) or forehead?

It completely depends on what's causing the headache. It's a very generic symptom. High blood pressure and low blood pressure both cause headaches, for example--and caffeine will help raise low blood pressure, but make high blood pressure much worse.

If you have a dependency, taking less is going to hurt worse for a short time. But the same applies to heroin, too; it doesn't mean the heroin isn't screwing with your body while you're taking it.

Whatever the cause, though, there's absolutely no harm in making sure you are properly hydrated. Drink more water.

I've heard sex gets rid of headaches AND reduces stress...it's worth a shot!
