I would expect it included both.
They were probably ranked in terms of sales so as to include the most common. Cough medicines would fit the bill of common and ineffective for example.

that is SO true. Why on earth do learned men and women tell patients to take OTC cough meds? For a cough med to be effective it needs to have a narcotic in it!
