I've absolutely no idea who coined the expression, but it's something I picked up from a book (as in a dead-tree-printed-version-which-looked-likely-to-sell-enough-copies-to-be-published book) a few years ago.

I rather hope it's not intended to be offensive, given that I've been using it so long.
