They usually just "report" direct quotes. They put their analysis at the top, and the complete context at the bottom. Sometimes, the only bit that is theirs is the headline.

Often, when a Limbaugh quote (for example) is brought up, people claim it was taken out of context. So MM includes the context. So people attack their funding. As if MM's funding somehow made Limbaugh say something stupid. Or FOX misidentify a scandal-plagued Republican as a Democrat.

Sanford didn’t cheapen the value of marriage, he was victimized by the cheapening of marriage.”"

Just think! A perfect marriage until those nasty gays wanted their rights! Wow! Most marriages are not nearly that perfect!
