Dunno, but I reckon there should be a washer just under the head of the bolt as well as inside the nut.
The elongated loop is for holding with your hand. The round loop is for fitting over something that needs to be pulled or lifted without being touched by the hand. A device for removing hot sterilised bottles from the steriliser? ETA: BigV. Probably some kind of sex toy.
