That's just how X-rays work, they can't show depth position.
However, it is certainly possible that the bottle is on top of, or even underneath, the patient. We just cannot tell from an x-ray.
I also noticed the shapes Kumbulu points out, and while some (the soft blurry patches at the top right, eg) are probably just soft tissue, the hard straight-lined shapes intersecting and to the right of the bottle have me very puzzled. I don't take them as being suspicious, though - if you were faking this shot why would you throw in a bunch of extra junk?
And Cloud is very right - if this is real it is a horrible, terrible thing. Most of the time I keep myself desensitized to the horrors of the world. They are so many and so bad that they'd be quite overwhelming and drive me to despair. Humor is a defense. I just have to guard against apathy and cynicism, though.
