Unless you don't go to the hospital, there's no lasting effect.
Otherwise, the first stage is pain that can become unbearable. Last stage is penile necrosis. The reduction in size is then drastic, more like amputation.
It is called priapism.
And I can tell from personal experience that it is very painful. At fisrt, I made quite an impression on my girlfriend. She drove me to the hospital after 6 hours. I was unable to drive. They injected me with something. Fortunately, no lasting effect.
