I've fallen from a great height. ive been buried alive. that was the worst.

Car crash. When I was younger I had dreams about it.

Shot in bed by a jealous husband, at the age of 104.

I was shot by Lee Harvey Oswald whilst in a parade in Dallas.

Burning ship and drown in a creek, face up, just below the surface.

I backed into a spinning airplane propeller.

If I use my dreams as a reference I also fell from a great height.
Into water.
I often dream I am on spindly, ill-made structures above the sea, which are swaying to and fro. Perhaps I was a sailor and fell from a crowsnest?

If dreams or an irrational fear are the indicators then I must have died in a house fire :P

Since the age of about 15 I've had this fear of fire. It tends to hit as I go to bed. I often (especially if in a troubled state of mind) head to bed with escape routes running through my mind. How to get out if I can't get downstairs? How to get the dog out? This shit sometimes keeps me awake. It's one of the reasons I fight falling asleep, I think.

I've always died in the now. So I don't know. There were never any instances of previous time or lives about it. Not that I can remember.
