What kind of Heart Problem is this?
 
I story and one of my characters has a heart problem that only effects him if he physical wears himself out causing shortness of breath

Could be a number of different things. Tell your character to see a cardiologist.

My angina works almost exactly opposite to how your character feels. The symptoms, mild or otherwise, only ever appear in the first few minutes of exercise and so long as I ease off a bit will disappear by the time I'm properly 'warmed up' - they don't return even when I'm working harder or nearly exhausted.
